Last Will and Testimate

This Simple Will form is for married people with two or more children. The form used to generate the Children's Trust is broken down into three parts. The first part gathers general information. The second part gathers information relating to appointing a guardian for your child(ren), and the last part gathers information relating to the disposition of tangible personal property.

Once you complete all three parts, a Last Will and Testimate that you can print. After you print the Last Will and Testimate, you will need to have it signed by two witnesses and notorized before taking it to your attorney. Bolded items are examples of completed fields from this multi-part form.
